We try to keep our practice small so that we can give you that 'old fashioned' personal medical care that cannot be provided in the "industrial sized" clinic. We try to individualize our care of your family, and we know who you are and what is important to you.

What to bring:
  • A copy of your driver license
  • A list of all your meds, dosages, frequencies
  • If you are not English proficient, an interpreter may help us to complete your examination.
  • Any pertinent labs (example: if you have Diabetes, please bring an a1c which is < 3 months old)
  • If you are diagnosed with ANY medical, nerve, psychological, muscle, bone or vision problems, please have a note from your Primary Care Physician indicating that you are fit for all duties as a CDL operator as it relates to any and all of your problems.
What to expect:
  • You will need to arrive 30 minutes prior to your examination to fill out your forms.
  • The requirement: the exam must be preformed with patients robed in underwear and gown.
  • The examination takes approximately 15 minutes.
